Guides and Templates included:
- Care Plan Template – This Care Plan helps you communicate everything a caregiver needs to know about your Loved One. Think of it as an instruction manual that helps your Loved One receive great care.
- Live-In Caregiver Agreement– This document outlines the live-in caregiver’s job responsibilities, pay, time off, and house operations.
- Family Commitment Agreement – An agreement between the families of the residents sharing the home about everyone’s responsibilities and costs. This agreement helps prevent misunderstandings about money, schedules, and responsibilities. It’s intended to clarify important matters that are not covered in the Residential Lease and/or the Caregiver Agreement.
- Caregiver Job Description – This document covers position summary, responsibilities, qualifications, job requirements and income/benefits package.
- Hiring a Live-in Caregiver- This document is designed to support families in hiring a live-in caregiver for a Shared living home. It outlines the roles, expectations, and recommends interview questions.
- Caregiver Lease Addendum- Clearly defines the caregiver’s terms of residency.
- Household Manual Template -This manual is our “house playbook.” It contains all the practical information everyone needs to live together successfully. Below are general guidelines to promote teamwork and communication between families and Caregiver(s).
- Housemate/Mentor Agreement -This document outlines the terms of a Shared Living Housemate/Mentor Agreement. It defines expectations, responsibilities, and boundaries within a shared living arrangement designed to support the Resident.
- Budget Spreadsheet – This spreadsheet identifies common expenses and how each resident contributes to the total household costs.
